Specialist Domiciliary Care

New Horizons Care offers a person-centred and holistic approach in specialist domiciliary support for a range of complex physical health conditions and acquired injury or disability. Our care can benefit the lives of those with acquired brain injury, spinal cord injury, Motor Neurone Disease, Multiple Sclerosis, and Muscular Dystrophy, as well as other cases. We ensure that our clients have the very highest quality of life and can aspire to and maintain an improved sense of independence, centring our service around individual needs.

As well as offering clinical services, we believe that supporting clients in aspects of daily living such as completing domestic tasks, assisting with fluid and nutrition management and accessing community activities all help to encourage and sustain a sense of self and identity.

The domiciliary clinical services we provide include:

  • Respiratory management such as non-invasive ventilation, cough assistance and oral suction.

  • Tracheostomy care including tracheal suction and changes.

  • Gastrostomy feeding.

  • Medication management.

  • Epilepsy Management and Buccal Midazolam Administration.

  • Stoma and catheter care.

  • Moving and handling, including hoisting.

  • All support is managed and overseen by our RGN Clinical Lead.

New Horizons Care management team work closely with healthcare professionals including Registered Nurse, Respiratory Physiotherapist, Dieticians, Speech and Language Therapists and Occupational Therapists. This ensures that all clients receive detailed individual care planning and input; which is regularly reviewed. Our Support Workers attend mandatory training but also attend training that is specific to the individual needs of our clients. Staff are assessed as competent and they receive ongoing clinical supervision in all aspects of a person’s care requirements.

Home Care

New Horizons Care is a reliable and established personal care-at-home service provider, ensuring the very highest level of support from our professional and expert team to individuals who may have learning disabilities, mental health difficulties, sensory impairment, Autism, ASD, dementia, elderly care or palliative care needs.

Tailored care, with personalised and empowering support within your own home promotes both empowerment and independence for our clients and their families.

Initially with a thorough care assessment process enabling a bespoke support plan to be built, with regular reviews and the ability to adapt and change care plans whenever required. We aim to achieve fulfilled and healthy lifestyles by supporting individuals to socialise, maintain positive relationships, and partake in community activities

We compile comprehensive and client-specific care plans which take into account each client’s physical and mental health requirements and honour their cultural, religious and lifestyle needs. Our client will receive a dedicated Service Manager to oversee and coordinate the care according to the ever-changing needs and ambitions of the individual, as well as meeting with family members and relevant stakeholders.

At New Horizons Care we understand that meaningful support requires comprehensive training, and so we provide our staff with ongoing professional development as well as client-specific training.

Clinical Support

At New Horizons Care, all clinical care requirements are overseen by a Registered Nurse, including the training of staff, review of risk assessments and processes and guidance. This allows us to provide exceptional ongoing support in a safe and secure environment.

The RGN provides clinical advice, which includes continence management, skin integrity, medication management, PEG feeding and tracheostomy care. Our RGN also provides supervision and competency assessments for our staff.
